Must be able to work in a stressful environment and take appropriate action. Performs and documents initial and ongoing assessments of patient's condition.     Registered Nurse (RN) Full -Time- Nights Ascension Saint Thomas Rehabilitation Hospital is a state-of-the-art, 40-bed inpatient acute rehabilitation hospital dedicated to the treatment and recovery of those who have experienced serious injury, trauma or illness. Your specially trained care team includes a medical director, rehabilitation-certified nurses, physical therapists, occupational therapists, speech-language pathologists, dieticians, respiratory therapists and more. We start by listening to better understand you and your health. Then, we create a personalized care plan that helps you regain your strength. JOB SUMMARY: Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develop nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process. QUALIFICATIONS: Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program. Current state licensure as Registered Nurse. CPR/BCLS certification. ACLS preferred. CRRN Preferred Minimum six months of Medical/Surgical experience in an acute care setting preferred. Excellent oral and written communication and interpersonal skills. ESSENTIAL FUNCTIONS: Accurately performs patient assessments and identifies patient needs. Identifies and initiates appropriate nursing interventions. Provides patient and family teaching throughout the rehabilitation stay and prior to discharge. Provides care appropriate to the condition and age of the patient. Works cooperatively and actively participate with other members of the interdisciplinary team. Participates in Continuous Quality and Performance Improvement (CQPI). Maintains positive relationships with all customers. Promotes rehabilitation nursing philosophy and scope of practice. Actively participates in team conferences. Performs timely and appropriate documentation relating to medical necessity in the medical record. Responsible for completion and revision of the Interdisciplinary Care Plan for each patient. Performs timely and accurate QI assessments. Provides a safe environment for patients, visitors and staff and complies with appropriate and approved safety standards. Supports the leadership for the hospital in a professional manner and acts as a role model. Other duties as assigned. How you'll contribute A Physical Therapist (PT) who excels in this role: Performs initial and ongoing assessments of patient's condition. Establishes a plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires. Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients. Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the patient. Directs and supervises physical therapy assistants, students, and other support personnel. Assesses competence, delegates specific tasks, and establishes channels of communication. Administers treatment involving application of physical agents, using equipment, moist packs, ultraviolet and infrared lamps, and ultrasound machines. Evaluates, fits, and adjusts prosthetic and orthotic devices and recommend modifications. Identifies and documents goals, anticipated progress and plans for reevaluation.
