Our patients are homebound and mostly elderly and ill. It is necessary to provide each patient with the time and attention necessary to meet his or her needs. Job Specific Accountabilities The statements below describe the general nature and level of work being performed in this role. Notes: The position will require travel within the city of Houston/Angleton as well as their contingent counties. Must work with other healthcare professionals to create treatment plans. You will be responsible for but not limited to monitoring patient's vital signs, wound care, foley care, monitor changes in behavior and progress, as well as monitoring and administers medication. Communication with a referring physician and participates in professional development activities will also be required. Skill level required will vary depending on patient needs and will include training on G-tube care, trach care, wound care and foley care as well as ADL's, and other skills. Seeking skilled nurses with the comfortability of working independently and familiarity with OASIS, for RNs.
