Scottsdale, Arizona Our client is NOW hiring for an RN Chronic Case Manager in the Scottsdale area. This position is Full Time with office hours of (8:00 am to 5:00 pm Monday - Friday) and is contract, long-term, possible permanent. Join our client's growing Case Management department! ****Hourly rate of pay is $55.00 an hour**** General Job Duties: Provide assessment, disease management, education, training, and other clinically based activities to coordinate care among providers, patients and the community Conduct admission reviews, post-discharge calls, and discharge planning Provide ongoing support and counseling to patients and their families, addressing their concerns Make adequate contact with the patients with a focus on behavioral goal setting and patient-centered support planning Identify patient resource needs, provides information on possible resources and referrals to patients General Requirements: A degree in Licensed Registered Nursing from an accredited college or university AND certification as a Licensed Registered Nurse (RN) required Arizona Registered Nurse license required Certification as a Certified Case Manager (CCM) Highly Desired Diabetes/Disease Management RN experience/Chronic Case Management Company DescriptionAPN Staffing and Employment Solutions is a recruiting firm specialized in working with clients and candidates in the healthcare and automotive industry.
