    Requirements

    ABOUT THE ROLE

    This is a professional nursing position which requires thorough knowledge of commonly used psychotropic and health maintenance medications, an understanding of the special medical needs of adults with severe and persistent mental illness, substance abuse conditions or cognitive disabilities who may have co-occurring substance use disorders, physical disabilities, debilitating medical conditions, developmental disabilities, or infectious diseases.


    RESPONSIBILITIES

    Facilitates health care service needs of individuals served and ensures that these service needs are addressed.Competes all required medical assessments pursuant to best practices, contractual obligations, and licensure requirements.Ensures that an individual's health care services adhere to the best practice, licensure requirements, and are consistent with the policies, procedures, and protocols of contractors and payers of services.Delivers effective wellness and medical services in accordance with policies and procedures; state and federal regulations; and extant professional, ethical, and legal principles.Maintains an effective therapeutic alliance with individuals served.Communicates and coordinates with individuals receiving services, healthcare providers, family members or legal guardian regarding individuals’ health care needs.Abides by state and federal regulations, policies, and procedures related to confidentiality, patients’ rights (PM 46), and exposure and infection control.Provides after hours on-call support to program staff.Encourages staff/clients while demonstrating self-initiative in problem solving.Assist in the development of data collection and analysis of the quality improvement projects.Ensures all client records are maintained according to agency standards, policies and procedures.Engages in professional development. Responsible for keeping current in the field. Completes seminars and/or continuing education programs, as appropriate to update knowledge about addiction and mental health services.Acts as liaison with outside agenciesPrepares and submits routine and special reports, inclusive of accurate data, payroll, etc., to supervisor via computer within specified time frames.Participates in the development of quality improvement activities designed to enhance agency and program servicesEngages in professional development.     REQUIREMENTS

    Delaware Licensed Practical Nurse in good standing or Compact License required.Knowledge of nursing practice and other regulations pertaining to the administration and storage of medications.Ability to make independent judgments in emergencies.Must have the ability to function independently and as part of a team in carrying out the treatment plans and physicians’ orders in community settingsPrevious experience working with persons who struggle with intellectual, developmental, or behavioral health challenges preferredAbility to pass a background check and drug testThree year Motor Vehicle Record and valid Delaware driver’s licenseCPR and First Aid CertifiedObtains all required training, skills, and certifications required by the organization, accreditors, regulators, and contractors

    Alternative Consulting Enterprises, LLC, an affiliate of Apis Services Inc., is seeking a part-time Psychiatric Nurse Practitioner to provide clinical services to children, adolescents and adults.


    POSITION STATEMENT:

    Under the direct supervision of the Psychiatrist, the Nurse Practitioner is responsible for quality and completeness of assigned medical services at any of the clinic sites, including providing medication management for assigned patients. Under the general supervision of the lead physician and in accordance with standardized procedures, the Nurse Practitioner is responsible for providing psychiatric care to ambulatory patients through regular mental health assessments and the management of common mental health conditions. The Nurse Practitioner obtains a health history and performs a physical assessment, identifies problems, and formulates plans that include treatment, health education and counseling. The Nurse Practitioner collaborates with other health care professionals in assessing and managing health care problems and maintaining continuity of care.

    RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS

    The duties of this position include, but are not limited to, the following:

    Provide psychiatric care to members, including histories, exams, lab tests, prescriptions and referrals within prescribed up to date guidelines and protocols.Perform accurate and timely documentation of histories, subjective findings, physical exam, plans, prescriptions and follow up in Electronic Health Records System within 72 hours of the visit.Document patient chart entries in progress note format; following ACE policies, and Medicaid and Medicare guidelines.Assure that health education is provided with each patient encounter when applicable.Provide required federal and states ambulatory patient care data on appropriate forms.Provide members and patients information concerning their evaluation, diagnosis, treatment and prognosisEngage members in participation and decision involving their healthcare. Provide counseling for patients requiring additional supportActively participate and engage in quality improvement for clinic efficiencies and improving member quality of care. Timely coordination of care among internal and external ancillary providers and hospitals to provide high quality care between departments and facilities.Consult supervising physician or colleagues when needed for complicated or case is outside the scope of practice.Advise the Medical Committee/ Facility Administration of any situations that may adversely impact the efficiency of the medical department.Safety: Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves.Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention.At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the CommunityOther duties as assigned by Supervisor.­

    REQUIREMENTS:

    Be able to see between 14-24 members per day, and complete accurate documentation within 72 hours of visit
