The RN promotes, restores, and maintains resident's health and wellbeing by collaborating with providers, the interdisciplinary team, and care associates. The RN's primary role is to provide physical and psych-social support to residents and families.
Compensation: $38-$49/hour, commensurate with experience, plus differentials.
Schedule: 2 weekdays, and every other weekend 3p-11p in SNF.

How you will make an impact
Developing relationships with the residents and their families using a consistent caregiver approach, supporting the resident's physical, spiritual, emotional, and clinical needs, and focusing on placing the resident first. Performing all clinical duties in accordance with the state nurse practice act and nursing standards of practice Promoting resident's independence by establishing resident care goals, teaching resident/family members to understand a resident's condition, medication, and self-care skills Monitoring a resident clinical and care conditions to identify any changes in status and acting on those changes to ensure patient comfort and safety Working alongside the care associates to perform routine nursing care to assigned residents as needed Maintaining infection prevention practices, administering medications, and performing treatments per orders. Developing and coordinating a comprehensive Holistic plan of care to meet the resident's clinical and care needs in collaboration with the interdisciplinary and neighborhood team Supporting resident's preferences, routines, and choices in resident's electronic medical record Ensuring that documentation is current, accurate, and timely. Participating in care conferences for designated residents as requested by Clinical ManagerWhat you will need
Must have a minimum of 6 months experience as an RN Experience in caring for seniors or senior with cognitive impairment preferred Basic computer skills required including experience with Microsoft Office, internet and web applications Experience with an electronic medical record is preferred Must have an active RN license in the state in which they will practice CPR certification required.Please note that specific state regulations and requirements may be applicable. How you will make an impact
Coordinating, implementing, and overseeing the clinical care of guests/residents in skilled nursing/long-term care neighborhoods Maintaining clinical practice and processes in accordance with the Erickson Senior Living Management Policies and Procedures and nursing standards of practice. Ownership of documentation process, including MDS, care plans, incident reporting, and charting requirements Ensuring appropriate staff is on duty based on guest/resident acuity and provides recommendations based on identified needs of the neighborhood. Monitoring all aspects of infection control for staff and guests/residents and collaborating with clinical leadership to develop an appropriate process to ensure effective infection control monitoring, including antibiotic stewardship. Analyzing, developing, and implementing Quality Improvement measures for clinical indicators/benchmarking such as Falls, Pressure Ulcers, Medication Errors, Restraint Utilization, Re-hospitalization, Anti-psychotic utilization, ADL splits/documentation, etc. Collaborating with Staff Development Coordinator to ensure all required training and competency reviews are completed according to federal, state, and local regulations. Participating in clinical leadership meetings such as Wound Rounds, Clinical Operations Meeting, Utilization Review, and Performance Improvement/Risk Management/Safety (PI/RM/S) Committee Provides leadership and guidance during annual survey/complaint investigations as well as other regulatory audits/reviewsWhat you will need

How you will make an impact
Responsible for compliance with state, local, and company policies, procedures, and regulations. Coordinates and provides clinical oversight and care to residents within Memory Care, including medication management, pain management, wound care, nutrition, and respiratory care. Partners with Memory Care Manager to complete resident pre-screening/admission, re-admission, an ongoing and significant change of condition assessments. Ensures resident's clinical assessments reflect the resident's preferences, goals, and individualized clinical needs. Ensures resident's individualized service plan is maintained/updated to reflect current needs in collaboration with other assisted living/memory care team members (ALM, care associates, therapists, etc.). Coordinates and monitors outside support services such as hospice, therapy, home health, etc., following established plans of treatments and ensures that plans of treatments are integrated into resident's individualized service plans per state and local regulations. Tracks and trends/analyze clinical quality areas within Memory Care including wound care, infection control, falls risk reduction, pain management, weight loss, and dehydration and develops corrective action plans, implements plan and evaluates results of plan identified areas of concerns. Actively participate in continuing care meetings, including Operations Meeting, Service Plan Meetings, Family Meetings, and Performance Improvement/Risk Management/Safety (PI/RM/S) Committee.Compensation: Commensurate with experience starting at $95k per year with potential for annual bonus of up to 8%.
What you will need
