    REHABILITATION COORDINATOR

    ASC Treatment Group

    NOTE: There are two (2) open Rehabilitation Coordinators to be filled in the North Hollywood, CA area.

    This is human services position in a licensed, Medi-Cal-certified residential facility serving severely mentally disabled adults. The person filling this position provides a variety of services to clients, under the direction of the Program Manager.

    Rehabilitation Coordinator Responsibilities and Duties:

    1. Schedule: Should be flexible to work weekend schedules at times and irregular shifts. However, a regular work week is either Monday through Friday, 10:00am-6:00pm OR Tuesday through Saturday, 10:00am-6:00pm. Hours can be flexible for both.

    2. Care and Supervision:

    Provide on-site monitoring, care, and supervision to mentally ill clients.Assists with development, implementation, and documentation of individualized client/resident needs and service plans.Help implement schedule of client’s/resident’s programs and services at facility.Observe and interact with clients and respond helpfully to their problems and needs.Provide emotional support and encourage clients with their personal goals and objectives.Provide crisis intervention.Perform emergency procedures, as needed.Assist with organizing and supervising group rehabilitation services daily.Provide modeling and instruction for clients in daily living skills such as: personal care, interpersonal relations, domestic skills, utilizing community resources, coping skills, and symptom management techniques, etc.Complete client progress notes in a timely manner daily.Achieve individual monthly unit production goals per county contract.Participate in staff meetings and training activities, as required.Report any unusual situations or problems to supervisor without delay.Ability to maintain confidentiality at all times.

    Other Skills and Abilities:

    Must be aware of and understand the needs of the clients/residents where appropriate.

    Rehabilitation Coordinator Qualifications and Requirements:

    Minimum Bachelor’s degree OR two years of experience providing services to mentally ill or other disabled populations desired OR a combination of education and experience.Valid California driver’s license, reliable automobile, and good driving history, as demonstrated by DMV record check.Current certification in CPR and first aid needed but not required.Be in good health and be physically, mentally and occupationally capable of performing duties, as verified by recent health screening and TB tests: Be of good character and have no criminal record, other than minor traffic violations, as verified by fingerprint and criminal background checks.Good written and verbal communication skills, personal characteristics of warmth, enthusiasm, sense of humor, responsibility, and respect for others.     This is a position for and in an outpatient clinic serving mentally ill adults. This position is based in the North Hollywood, CA area (91601).


    Schedule: Monday through Friday, typically 8:00am - 4:00pm. May be required to work flexible hours, irregular shifts, and at different locations.


    Care and Supervision: Provide transportation for our mentally ill clients from their board and care residences to our outpatient clinic and back. Help implement and coordinate schedule of resident's/client's programs and services at the facility.     Essential Duties and Responsibilities:Inspect vehicles dailyCheck tire gaugescheck for cleanlinessCheck fuel levelCheck mileage sheets and maintain mileage logs (for completeness and accuracy)Maintain general maintenance of all ASC vehicles utilized by Valley Vista and North Hollywood and Sunland Manor staff.Participate in gathering ASC clients for transport to our day program, doctor's appointments and other appointment services.Assist clients to enter and exit the vehicle safely.Assess for client appropriateness to travel and engage staff to escort clients back into the facility when they are not appropriate for safe travel.Double-check that all clients are accounted for.Organize, plan and execute transport of clients to community outings.Assist with client lunch and cleanup daily at the day program.Including picking up food, preparing simple meals, and plating meals.Assist with running errands/courier activities between company sites, when needed.Qualifications and Requirements:Minimum High School Diploma and two years of experience providing services to mentally ill or other disabled population desired.Valid California Class B driver's license (with Passenger Endorsement) and good driving history, as demonstrated by DMV record check.Current certification in CPR and first aid (we can assist with this if needed).Be in good health and be physically, mentally, and occupationally capable of performing duties, as verified by recent health screening and TB tests.Be of good character and able to pass a criminal background check.Personal characteristics of warmth, enthusiasm, sense of humor, responsibility, and respect for others.Current De-escalation Training certification, preferred but not required.

    Summary: This is a full-time human services supervisory position based in an outpatient clinic serving severely mentally ill adults. The person filling this position will provide a variety of care and supervision services to residents under the direction of the Executive Directors, Medical Director, and the Clinical Director.

    Essential Duties and Responsibilities include the following. Other duties may be assigned.

    Coordinate with direct care staff in the performance of their duties.Assist with development and implementation of individual client needs and service plans.Utilize and coordinate medical services provided at the facility and by outside agencies.Organize and coordinate nursing educational opportunities for staff and residents.Provide modeling and instruction for residents in Adult Daily Living Skills (ADLs), coping skills, utilizing community services, and self-care.Schedule and coordinate transportation for resident’s medical, dental, and other appointments. Employ best nursing practices in documenting appointments.Ensure annual psychiatric assessments are complete and timely.Administer management of injections and maintain a positive medical administrative record (MAR).Prepare and complete documented coordinated services with follow-up as required including developing medication goals initially, then annually.Participate in staff meetings and training activities as required.Assess clients as required for physical, mental, and side effect changes in status.Ability to document services, progress notes.Make every attempt at crisis intervention, critical thinking and problem solving with residents.Maintain and promote healthy boundaries between staff and residents.Report all repairs and concerns to administration immediately.Possess a clinical understanding and follow an Integrated Resident Care Plan as well as function as a member of the multidisciplinary team treatment plan with responsibilities that include:Able to facilitate an integrated cohesive, coordinated, and supportive treatment approach.Assist in the admission process and preparation of all required forms for clients accepted into the program, including initial intakes and treatment planning.Coordinated with all persons involved in the client’s treatment to include but not limited to the family, case manager, treatment coordinator, therapist, etc.Have knowledge of various psychotropic medications and their effects.Monitor clients and provide educational treatment services related to substance use disorders, physical conditions (DM, metabolic syndromes, personal healthcare, diet, obesity, hypertension, dyslipidemia, and behavioral habits, etc.).Must be able to manage the following:Planning client’s medical appointments with medical director and any other outside physician.Follow up on all outside physician appointments and report results to Medical Director.Identify clients who require monthly and/or daily vital signs.Make arrangements for lab drawings to facilitate services in-house and attend when services are available to clients.Review monthly menus and incorporate USDA guidelines in balanced meal recommendations.Make suggestions for cost effective purchases.Provide support to other staff including other departments, as needed.Other duties as assigned by the Executive Directors, Admissions Coordinator, and Program Manager Supervisor.

    Directly supervise Nursing Administrative Assistant along with training duties that include:

    Assisting with scheduling, planning outings and activities for the residents.Set goals for performance and deadlines in ways that align with Company’s vision and communicate them to subordinates.Assist with organizing workflow and ensure that employee understand their duties.Monitor employee productivity and provide constructive feedback and coaching.Receive complaints and resolve issues that may arise.Relay information from management to employee and vice versa.Develop and manage schedule along with Admissions Coordinator to inform employee of any business changes, if needed.Coordinating client medical services as well as transportation to appointments.Assist with coordinating administrative duties such as: finger sticks, documenting services, ongoing recordkeeping, and maintaining medical records.

    Education and/or Experience

    Bachelor's degree (B. A.); and/or two years related experience and/or providing services to the mentally ill; or equivalent combination of education and experience.

    Computer Skills

    Basic computer skills, including being able to access data and print documents.
    Intermediate skills in Microsoft Office: Word and Excel.

    Certificates, Licenses, Registrations

    Valid CA driver’s license and proof of automobile insurance.Completion of CPR/First Aid and Pro-ACT certification within the first thirty (30) days of employment.Licensed by State authority as a Licensed Vocational Nurse or Licensed Psychiatric Technician.
    Typically, 8:00am-4:00pm but can be flexible within reason.
