This 13 week replacement assignment runs on a Tuesday Saturday day shift schedule, focusing on functional recovery, mobility, and safe discharge planning. Assignment details Location: Skilled nursing/rehab facility, Pleasant Hill, CA 94523 Setting: Skilled Nursing Facility (SNF) / post acute rehab with primarily geriatric caseload Position type: Contract, full-time, non permanent Contract length: 13 weeks (03/09/26 06/06/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break Days: Tuesday, Wednesday, Thursday, Friday, Saturday (Tues Sat schedule) Guaranteed hours: 36 hours per week Gross Blended Pay Rate: $60 - $72 per hour Order reason: Replacement Key responsibilities Complete comprehensive PT evaluations including strength, ROM, balance, gait, and functional mobility for adult/geriatric patients. Develop and implement individualized plans of care to improve transfers, ambulation, stair negotiation, and overall functional independence. Provide evidence based PT interventions such as therapeutic exercise, gait training, neuromuscular re education, and balance/coordination activities. Collaborate with the interdisciplinary team (OT, SLP, nursing, social services, physicians) to coordinate care and discharge planning. Supervise PTAs and rehab aides per California practice regulations and facility policy. Maintain accurate, timely documentation of evaluations, daily treatment notes, progress notes, and discharges in the EMR. Educate patients and caregivers on home exercise programs, use of assistive devices, safety, and fall prevention strategies. Required qualifications and skills Current California Physical Therapist license (or eligibility prior to start). Graduate of an accredited PT program (DPT, MSPT, or BSPT accepted). Prior experience in SNF, subacute rehab, or similar post acute setting strongly preferred. Current BLS/CPR for healthcare providers. Strong clinical reasoning, documentation, and time management skills with the ability to manage a full SNF caseload.
