    DUTIES


    1) Performs Occupational Therapy assessments of patient functioning levels, plans short and long term goals toward improving/restoring independence and physical/cognitive functioning; and implements developed treatment programs, approved by the attending physician. Conducts OASIS assessments at appropriate time points

    2) Provides guidance and instruction to patients/family members of developed therapeutic, creative, and self care procedures/activities; and explains adaptive and progressive approaches in applying it.

    3) Instructs the patient/family members in use of appropriate orthotic or prosthetic equipment; and may also design and fabricate customized adaptive and assistive devices, with training for their use.

    4) Conducts functional and cognitive re-evaluations, as indicated, to assess the need for any revisions in the treatment plan.

    5) Instructs and familiarizes, as necessary, the patient and pertinent family members with all other aspects of the total treatment program.

    6) Maintains adequate clinical and progress notes and submits all required paperwork in a timely manner according to Agency policy.

    7) Periodically reports to the physician on the patients treatment, response, and progress, and assists the physician in determining any necessary changes in the treatment program.

    8) Arranges for outpatient occupational therapy services that cannot be rendered in the home, as necessary.

    9) Provides consultation to, and instructs other members of the home health team in the use of necessary equipment, techniques, modalities and basic activity programs, when needed, and as requested.

    10) Maintains and updates professional competency through in-service attendance and similar professional resources.

    11. Maintains communication with the Physical Therapist and/or Nurse/Case Manager.

    12. Participates in staff meeting and case conferences as required.


    CRITICAL DEMANDS


    1) Must be able to meet the physical demands of providing occupational therapy services, such as ambulating patients, transferring patients, assisting with exercises, etc.

    2) Must be able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety.


    QUALIFICATIONS


    1) Must possess a Bachelor of Science or Masters degree in Occupational Therapy from a school with an Occupational Therapy curriculum accredited jointly by the Council of Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association.

    2) Must hold a license/registration as an Occupational Therapist, issued by the State of Michigan.

    3) CPR certification preferred

    4) Current Michigan drivers license and/or reliable transportation with proof of automobile insurance of personal vehicle for business use.

    5) Must be empathetic, non-judgmental, tactful, responsible, and organized.

    6) Must have emotional and mental maturity necessary for establishing and maintaining good work relationships with patients and families.
