    Job DescriptionJob DescriptionAccura Health is looking for a PRN/Full Time Nurse Practitioners (NP) with a valid TEXAS NP license in good standing with the State.

    The position will entail working with complex post acute care patients for evaluation and follow-ups in the home. These visits will be a combination of Transitional Care (TCM) and/or House Calls.

    NP will work within an assigned territory or coverage area.

    Requirements

    Qualified candidates must be Nurse Practitioners with a valid Texas Nurse Practitioner license in good standing with the stateResponsibilities

    Performs routine and emergency assessments:Records observations, treatments, and other pertinent information.Communicates with Medical Director, Ordering Physician, Care team and internal office team regarding findings from the visits and any measures to alleviate symptoms.Provides physical and emotional care to support the patient and family in period of crisis.Prevents unwarranted hospitalizations by meeting patient and family needs in the home.Educates patient and caregiver regardingOrders, performs, and interprets laboratory and radiology tests within scope of professional practice.Prescribes medications including controlled substances to the extent delegated and licensed.Orders treatments and durable medical equipment as indicated.Performs other therapeutic or corrective measures as indicated.Consults with physician or designees as needed, informs primary physician of services provided and collaborate with other physicians as needed.Assists in all facets of care coordinationPrepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.Complies with applicable laws and regulations with respect to Collaborative Agreements.Initiates reimbursement for services rendered, provides training and continuing education for staff.Assists in development of clinical practice guidelines/standards in support of quality palliative care.Defines goals for professional growth and participates actively in professional activities and organizations.Engages in active and frequent self-care activities for personal and professional growth and longevity.Adheres to the practice of confidentiality regarding patients, families, staff and the Organization.Supervisory Responsibilities:

    Supervision of nursing trainees, including graduate and advance practice nurses in field placements.Participation in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees.Performs other duties as assigned. Read Less

    Position Summary: We are seeking experienced and highly motivated Nurse Practitioners, Physician Assistants, Medical Doctors, and Doctors of Osteopathic Medicine to join our growing family practice. Candidates must possess strong clinical skills and a passion for delivering high-quality primary care services. This role will involve managing a broad spectrum of acute and chronic medical conditions, preventive care, and patient education.

    Key Responsibilities:


    Provide comprehensive primary care to adult and pediatric patients in a family practice setting.

    Perform physical examinations, diagnose illnesses, develop treatment plans, and manage chronic conditions.

    Order and interpret diagnostic tests such as labs and imaging.

    Prescribe medications and manage medication regimens in compliance with DEA and state regulations.

    Collaborate with a multidisciplinary team, including medical assistants, nurses, and other healthcare professionals.

    Educate patients and families on health maintenance, disease prevention, and treatment options.

    Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system.

    Participate in quality improvement initiatives and adhere to evidence-based clinical guidelines.

    Qualifications:


    NP/PA Candidates: Masters degree in Nursing or Physician Assistant Studies with a valid license to practice as an NP or PA in the state.

    MD/DO Candidates: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree with an active state medical license.

    Experience: Minimum of 3-4 years of clinical experience in primary care, family medicine, or internal medicine.

    Certification: Board certification in family practice or primary care specialty.

    DEA License: Required for prescribing controlled substances.

    Proficiency in managing a wide range of primary care conditions for both adult and pediatric patients.

    Strong diagnostic and clinical decision-making skills.

    Excellent communication and interpersonal abilities.
