Responsibilities:
Provide direct, hands-on personal care and daily living assistance to clients in home and community settingsSupport clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparationAccompany and assist clients with community outings, appointments, and social activitiesMonitor and document client health, behavior, and progress, reporting changes to supervisorsImplement individualized care plans tailored to each client's unique needs and goalsFoster a safe, supportive, and dignified environment for all clientsBuild trusting relationships with clients, families, and care team membersRequirements:
Schedule: 8:00am-2:00pm M-F
Family prefers female caregiver and dog friendly personPrior experience in home care, personal care assistance (PCA), direct support, or a related caregiving role preferredGenuine compassion, patience, and dedication to supporting individuals with diverse needsAbility to pass a background check as required by Minnesota state regulationsValid driver's license and reliable transportation preferredStrong communication and interpersonal skillsAbility to work independently and as part of a collaborative care teamAbout Us:
