This role will be responsible for providing Home Health care to patients in the local area in the comfort of their own home!
Responsibilities
Conduct comprehensive patient assessments and develop individualized care plans.Provide compassionate care by conducting health assessments and crafting personalized treatment plans.Administer and monitor medications while educating clients and staff on proper care.Utilize your experience with clinical assessment, psychosocial rehabilitation, and bio-psychosocial therapies to make a difference.Use your knowledge of electronic health records and data analytics to ensure optimal care and wellness outcomes for our clients.Requirements:
1+ year of professional experience as an RN (Home Health experience preferred)Active RN License in the State of PA Associates Degree (or higher) in NursingBLS and CPR Certification (if you don't have this - we will help you get it!)Valid Driver's License, Reliable Mode of Transportation, and Auto InsuranceSalary: $80,000 annually. This position is full-time.
