     Albright Life Centers are expanding our Nurse Practitioner Team! Come join our team of advanced practice professionals led by a knowledgeable, compassionate Medical Director. This is a great opportunity to grow your career! 

     Full-time: Monday-Friday, 8am-5pm with rotating on-call 

    This position rotates between Albright LIFE's Lancaster and Lebanon clinic locations.

    LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their healthcare needs in the community instead of going to a nursing home or other care facility. At an Albright LIFE location, you’ll work with a team of healthcare professionals providing exceptional, coordinated services each day that help participants stay healthy and safely live independently at home.

    Job Description

    Ensures ongoing coverage and continuity; coordinates with other physicians for on call coverage and participates in that coverage as needed.Acts as medical liaison between LIFE program and others in the medical community (i.e. primary physician groups (attending), specialists and hospital staff); promotes discussions regarding medications, treatments, discharge and alternative regimens.Ensures compliance with LIFE provider regulations as they pertain to medical/clinical components.Utilize your advanced clinical skills to accurately diagnose and treat a wide range of medical conditions, helping patients on their journey to recovery and wellness.Compassion, patience, and genuine desire to make a positive impact on the lives of seniors.Qualifications

    Nurse Practitioner with MSN and licensed to practice in the Sate of PA.Valid Pennsylvania Driver’s License.Minimum of one year of experience in care of a frail or elderly population.Experience working with an interdisciplinary team and performing comprehensive geriatric assessments preferred.Demonstrates leadership abilities and the ability to coordinate and facilitate teamwork and interdisciplinary groups. Demonstrates positive interpersonal relations in dealing with others, so that productivity and positive interpersonal relations are maximized.Consistently demonstrates and incorporates principles of safety and infection control into daily activities.

    Job Description

    Administer physical therapy screening, evaluation, and treatment to participants, ensuring personalized care aligned with their needs and goals in the clinic and the participants' homes.Collaborate with interdisciplinary teams to develop and implement comprehensive care plans tailored to each participant.Document participant progress accurately and comprehensively, utilizing state-of-the-art technology for seamless record-keeping.Commit to ongoing professional development by obtaining CEUs in accordance with state requirements.Qualifications

    Graduate degree from an APTA accredited Physical Therapy ProgramCurrent Pennsylvania Physical Therapist licenseValid Pennsylvania Driver’s LicenseOne year of experience working with a frail or elderly population

    Deliver competent and safe transportation services to our participants, maintaining a clean driving record without any safety-related accidents or moving violationsProvides transportation and related services for special events, trips and medical appointments. Ensures timely pick up and return.Operates any assigned vehicle in a safe and legal mannerAssist participants into and out of vehicle and wheelchairs and assist wheelchair bound residents between vehicle and destination.Never leaves vehicle running or leaves keys in vehicle while unattended.Inspect, maintain and safely operate wheelchair transport vehicles.Qualifications

    High school diploma or GED equivalent, required.Valid Driver's license, required.1 year experience working with the senior population, required.
