    JOB DESCRIPTION SUMMARY

    Responsible for managing all aspects of organization marketing including managing the members of the marketing team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and negotiating service contracts with managed care organizations, government agencies, and other payers.

    ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

    1. Directs all daily marketing operations including providing direct oversight of the establishment and implementation of marketing initiatives.

    2. Assures that staff understand and adheres to all laws, statutes and regulations regarding anti-kickback, lawful marketing activities (gifts and services), patient solicitation, and accurate marketing materials.

    3. Ensures maximum third party reimbursement through effective negotiation of service contracts with managed care organizations, government agencies, and other payers.

    4. Assists the Administrator in establishing organization volume projections in the annual budget and in establishing allocations for the marketing department. Monitors allocation of resources according to budgetary limitations.

    5. Continuously conducts market assessments and develops a comprehensive marketing plan designed to meet budgetary volume projections.

    6. Employs marketing and promotional initiatives to achieve budgetary volume projections.

    7. Establishes and maintains positive working relationships with current and potential referral and payer sources.

    8. Negotiates service pricing with insurance Case Managers and other payers within established financial and credit parameters.

    9. Recruits, selects, orients, and directly manages members of the marketing team.

    10. Builds and monitors community, customer, payer and patient perceptions of AFFINITY HOSPICE LLC as a high quality provider of services.

    11. Provides leadership in strategic planning including identifying opportunities for additional or improved services to address customer needs.

    12. Maintains comprehensive working knowledge of AFFINITY HOSPICE LLC markets including government agencies, major payer groups, key referral sources, and competitors market positioning.

    13. Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.

    14. Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by AFFINITY HOSPICE LLC.

    15. Monitors and reports cost effectiveness of marketing efforts.

    POSITION QUALIFICATIONS

    Job Purpose
    The MSW provides support to the patient and their family in accordance to the plan of care. The MSW assumes responsibility for the development, implementation, and management of psychosocial services. Being available to help navigate the practical and emotional issues that arise during the hospice journey. Provide support to complete advance directives, funeral arrangements, education, connections with community resources, and supporting bereavement needs.

    Core Duties
    1. Provide coordination to ensure continual assessment and personalization of each patients and familys needs from admissions throughout the hospice stay while collaboratively working with others to achieve patient, personal, and Company goals.
    2. Understand and abide by state/federal/Medicare/ACCREDITATION hospice regulations, HIPAA/PHI regulations, Office of Civil Rights, and Company policy and procedures.
    3. Conduct a comprehensive psycho-social assessment of the patient and family and participates in the development of the plan of care at the time of patient's admission. Ensure a complete assessment of emotional factors related to terminal illness. Identify family dynamics and communication patterns.
    4. Collaborate with the Interdisciplinary Group (IDG) members and physician in preparing, evaluating, integrating, and implementing the IDG individualized plan of care to assure the highest quality of social services are offered to patient and family in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness. Continually evaluates and reports and documents the outcome of services. Revise plan of care as needed. Involve the patient/family in the plan of care. Services provided are under the direction of a physician.
    5. Identify the development level of patient/family and obstacles to learning or ability to participate in care of patient. Address patient/family questions and issues. Identify obstacles to compliance and assist in understanding goals of interventions, support systems available to reduce stress, and coping with end-of-life care.
    6. Assess and strengthen the patient and family's coping skills. Assess the patient/family psychosocial status, potential for risk of suicide and/or abuse of neglect. Evaluate patient/family response to psychosocial interventions.
    7. Assess environmental resources and obstacles to maintain safety. Assess caregivers ability to function adequately.
    8. Provide interventions for specific symptom relief of fear, grief, depression, anger, etc. Assess special needs related to cultural diversity including communication, space, role of family members and special traditions.
    9. Document problems, psychosocial assessment, appropriate goals, care provided, interventions and family/caregiver response to interventions as outlined by hospice Policy and Procedures.
    10. Deliver social services to the patient and/or family by developing/maintaining working relationships with community medical/health care services. Assists the patient/family with identifying and accessing community resources and financial resources.
    11. Maintain appropriate documentation in the hospice patient chart and reports any emotional, social, or financial changes of the patient or family physician(s) and IDG.
    12. Evaluate long-term care when appropriate and assess ability to accept change in level of care.
    13. Work with management on level of care changes, discharge planning, and identify potential patient/family needs.
    14. Evaluated patient/family response to interventions when referred to community agencies and satisfaction of the services provided.
    15. Assess bereavement needs and coordinate follow up with bereavement services.
    16. Provide services that include short-term individual counseling, crises intervention, assistance in providing information and preparation of advance directives, and funeral planning issues and transfer to responsibilities regarding fiscal, legal, and health care decisions.
    17. Complete all charting within 24hrs of patient visit.
    18. Participate in on-call needs, admissions, routine visits, death visits to meet patient and Company needs.
    19. Participate in continuing education programs.
    20. Communicate with coworkers and management in a productive, respectful, and factual manner. Being mindful of tone, body language, and words used.
    21. Represent the Company in a professional manner by being mindful of behavior and what is communicated online or in person written or verbal format (ex. meme), wearing business casual/scrubs, being compliant, and be great at your job.

    Minimum Requirements
    Education & Experience
    1. Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.
    2. MSW Hospice experience 0 to 10 years.
    Knowledge, Skills, and Abilities
    1. Read, write, and speak English.
    2. Work independently and under the direction of the physician and management.
    3. Knowledge of community resources including Medicare / Medicaid, financial aid, county, state, and private social services agencies.
    4. Drive self to and from patient visits and the office.

    Physical Requirements
    Continuously: in a changing work environment (wet/dry/hot/cold), operating technology, completing charting, and in.
    Frequently: sitting, standing, walking, pushing and pulling up to 50lbs, and driving a vehicle.
    Occasionally: pushing and pulling over 50lbs and up to 100lbs, squatting, bending, kneeling, reaching, twisting, operating in tight spaces.
    Rarely: Pushing and pulling over 100lbs, crawling, climbing, walking on rough ground, exposure to dust/fumes/gases, being near moving equipment, operating medical equipment
    Never: Working from heights or working underground.

    Working Conditions
    Job requires flexibility in schedule to meet patient needs. Mainly working during daytime hours, but overnight or on-call hours may be necessary. Weekends and holidays may be necessary. Position requires travels causing exposure to temperature changes, weather changes, and traffic changes. Hospice can also expose staff to stress, depression, burnout, fatigue, long hours charting, and hauling supplies in the car.

    Job Purpose
    The RN Case Manager (RN) is part of the hospice nursing team and member of the Interdisciplinary group (IDG) that coordinates the care of the patient and family after identifying the physical, psychological, social, and spiritual needs of both. The RN initiates appropriate intervention and support for the patient and family upon admission and provides continuously appropriate, comprehensive, individualized, and responsive care plan. The nurse may also provide on-call visits, admissions, or deaths visits.

    Core Duties
    1. Provide program coordination to ensure continual assessment and personalization of each patients and familys needs from admissions throughout the hospice stay while collaboratively working with others to achieve patient, personal, and Company goals.
    2. Understand and abide by state/federal/Medicare/ACCREDITATION hospice regulations, HIPAA/PHI regulations, Office of Civil Rights, and Company policy and procedures.
    3. Provide initial, comprehensive and ongoing assessment of the impact of the terminal diagnosis on the patients physical, functional, psychosocial, and environmental needs and activities of daily living within the time frames as defined in the hospice regulations, company policies and defined in the plan of care. Assessments will include understanding the risk for pathological grief, cultural and spiritual implications, and verbal and non-verbal communication patterns.
    4. Responsible for the coordination of care and ensuring continuous assessment of each patient's and family's needs and implementation of the interdisciplinary plan of care. Work with the IDG to develop, provide, integrate, and implement individualized written plans of care for patient and family and recommend revisions to the plan of care as necessary. Ensure the plan of care manages discomfort and provides symptom relief related to palliative and end-of-life care. The plan of care will be reviewed and updated as necessary to meet current patient care needs, no more than every 15 days.
    5. Educate patient, family and primary care givers in the support of the patient including the disease process, self-care techniques, end-of-life care, and the processes for dealing with issues of ethical concerns.
    6. Utilize the skills and resources of contracted personnel such as PT, ST, OT, Dietary, Pharmacy, and other services when deemed appropriate for patient outcomes.
    7. Work with the patients physician(s) to manage care, intervention needs and outcomes, and receive orders when necessary.
    8. Prepare and chart clinical notes that demonstrate progress toward established goals, successful or unsuccessful use of nursing interventions/medications/durable medical equipment/supplies/diseases process/family interventions. Utilize approved scales/medical terms/medical acronyms/labs to complete charting. Review previous charts from nursing and all disciplines to ensure full understanding of patient care.
    9. Complete all charting within 24hrs of patient visit.
    10. Supervise and work with licensed practical (vocational) nurses (LPN/LVN)/certified nursing assistants/social workers/Chaplains to establish appropriate levels of care and meet patients individualized plan of care and needs.
    11. Participate in on-call needs, admissions, routine visits, death visits to meet patient and Company needs.
    12. Participate in continuing education programs.
