Job Responsibilities: Provide in-home physical therapy services to patients recovering from illness, surgery, or injury, helping them regain mobility and independence in the comfort of their own home. Conduct comprehensive assessments of patients' physical abilities, mobility, strength, balance, and functional limitations to develop personalized treatment plans. Design and implement therapeutic interventions, including exercises, gait training, and mobility techniques, to improve patient strength, coordination, and range of motion. Educate patients and their families on home exercise programs, techniques for improving mobility, and safety measures to prevent falls or injuries in the home. Collaborate with physicians, nurses, and other members of the healthcare team to ensure a coordinated and comprehensive approach to patient care. A minimum of 2 years of experience as a Physical Therapist (PT), preferably in a home health or rehabilitation setting.
