We aim to offer compassionate care to children and pregnant women, connecting them with vital community resources while promoting holistic well-being. Job Summary We are seeking certified doulas to join our team as Case Managers . This role focuses on supporting children and pregnant women by providing case management services, including linking them to community resources, coordinating care, and advocating for their needs. Eligibility Requirements To qualify for this position, applicants must meet one of the following pathways: Pathway 1: A doula who is certified in alignment with nationally recognized standards and, as determined by HHSC, has: Five years of experience within the last seven years as a doula. Attendance at three births within the last seven years. At least three written professional letters of recommendation . Pathway 2: A doula who does not meet the five-year experience requirement but is certified in alignment with nationally recognized standards and, as determined by HHSC, has: Attendance at three births . At least three written professional letters of recommendation. Training in the following core competencies: Childbirth education. Lactation support , or proof of being a certified lactation counselor (CLC) or International Board-Certified Lactation Consultant (IBCLC). Nonmedical comfort measures , prenatal support, and labor support techniques. Recognizing and responding to chronic and acute health conditions during the perinatal period. Cultural competency . DO NOT Apply if you do not have one of the following certifications. Your application may be automatically denied. Recommended Certifications: DONA International (Doulas of North America) CAPPA (Childbirth and Postpartum Professional Association) ICEA (International Childbirth Education Association) ProDoula ICEA-Approved Organizations Birth Arts International The Matrona Lamaze International Key Responsibilities Conduct initial assessments to determine the needs of children and pregnant women enrolled in the C.A.R.E.S. program . Develop individualized care plans in collaboration with clients, outlining goals, objectives, and interventions. Coordinate access to healthcare services, including medical, dental, and behavioral health care, as well as prenatal care for pregnant women. Proficient as a birth or postpartum doula. Knowledge of working with those battling miscarriages, stillbirths, etc. Able to work with entities to provide resources to mothers. Advocate for clients and ensure they receive timely and appropriate services from community resources, including but not limited to housing, transportation, food assistance, and childcare. Monitor client progress and reassess needs regularly, adjusting care plans as necessary to promote positive outcomes. Provide navigation opportunities and local education to clients on topics such as healthy lifestyle choices, prenatal care, parenting skills, and accessing available resources. Maintain accurate and up-to-date documentation of client interactions, assessments, care plans, and progress notes in accordance with program requirements. Collaborate with interdisciplinary teams, including healthcare providers, social workers, educators, and community organizations, to ensure coordinated and holistic care for clients. Participate in program meetings, trainings, continuing education, and quality improvement initiatives as required. Adhere to all program policies, procedures, and ethical standards, maintaining client confidentiality and privacy at all times. Knowledge of Texas healthcare and social service systems, including Medicaid and CHIP, preferred. Strong interpersonal skills with the ability to build rapport and establish trust with clients from diverse backgrounds. Excellent communication skills, both verbal and written, with the ability to effectively advocate for clients and collaborate with interdisciplinary teams. Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment. Proficiency in computer skills, including Microsoft Office and electronic health record systems. Bilingual proficiency in English and Spanish is preferred. (Incentive if Bilingual) Additional Requirements Employment is contingent upon successful contracting with HHSC/TMHP and at least two Managed Care Organizations (MCOs). Candidates may begin participating in the Lead Source Split Incentive Program while awaiting contracting completion. Job Summary AEIC Case Management is seeking certified Community Health Workers (CHWs) to serve as Case Managers within the Children Pregnant Women (C.A.R.E.S.) program. CHWs provide Medicaid-compliant case management services focused on care coordination, resource navigation, client education, and advocacy. This is a primarily remote position ; however, staff must reside in the DallasFort Worth (DFW) area , possess strong knowledge of local community resources , and be available for community-based activities as needed. Important Notice: Hiring does not result in immediate client assignment. CHWs may not begin seeing clients or billing for services until all required HHSC/TMHP and Managed Care Organization (MCO) contracting is fully completed and approved.
