    Review patient's history and perform examination. Evaluate, diagnose and provide appropriate treatment and patient care plan. Refer patient to primary care physician or other specialist as needed

    Provide clinical diagnostic skills in the delivery of patient care. Review test results and recommend suitable clinical management for the patient, including but not limited to, preventive health recommendations.

    Prescribe medications and formulate ongoing treatment and disease management plan.

    Collect, record, and maintain patient information, such as medical histories, reports, and examination results. Complete and insure accuracy of patient medical record.

    Communicate effectively with the clinical team to insure total delivery of quality care.

    Education, Training, Licenses, Certifications

    Doctor of Medicine (MD).

    Completion of four year post graduate medical training from an accredited cardiology residency program.

    Board Eligible/Board Certified in Cardiology

    Must possess current, unrestricted New York State license or be eligible for licensure.

    Must possess Drug Enforcement Agency (DEA) registration or be eligible for registration

    Relevant Work Experience, Knowledge, Skills, and Abilities

    Must possess Medicare/Medicaid Enrollment or be eligible to enroll. (R)

    Must possess strong communication skills both oral and written. (R)

    Proficient in the use of Electronic Medical Record (EMR) system and Microsoft Office. (R)

    Pay Range: $400,000.00-$575,000.00

We provide a good faith estimate of the salary range for potential hires, which is based on key factors such as role responsibilities, candidate experience, education and training, internal equity, and market conditions. Typically, new hires may not start at the top of this range, as compensation is tailored to each individual's circumstances. For union positions, salaries will be determined according to the collective bargaining agreement.     Job Summary

    We are seeking a practicing Internal or Family Medicine trained physician to join us in pursuit of high-quality patient care to provide a full scope of primary care services including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance.

    Must have a minimum of two-three years practice experience in similar outpatient setting

    The Ideal Candidate:

    Must have a minimum of two to three years of primary care experience in an outpatient setting.

    Ability to diagnose and treat a wide variety of chronic conditions and improve patient outcomes.

    Demonstrate competency in diagnosis and treatment to assure high standards of patient care and safety are maintained.
