We are currently seeking a Registered Nurse (RN) Case Manager to join our dedicated team and help patients remain safe and healthy in the comfort of their homes in Summit County.

We are looking for a compassionate and dependable RN who:
Holds an active RN license in Ohio with at least 1 year of experience. (Home health or geriatric experience is a plus, but not required.)Communicates effectively and professionally with patients, families, and interdisciplinary team members.Demonstrates strong organizational skills and the ability to manage a flexible schedule independently.Values teamwork and is dedicated to delivering high-quality patient care.What Our Nurses Say

Deliver physical therapy services under the supervision and direction of a licensed Physical Therapist.Implement individualized care plans to restore function, improve mobility, and promote independence at home.Collaborate with patients, families, and interdisciplinary teams to provide patient-centered care.Document treatments, progress notes, and communications accurately and in compliance with regulations.Participate in case conferences and team meetings to ensure coordinated, high-quality care.Qualifications
Active PTA license in Ohio (required).1+ year of recent PTA experience in a healthcare setting (hospital, SNF, ALF, hospice, or home health).Home health experience preferred, but we will train the right candidate.Strong communication, interpersonal, and organizational skills.Self-motivated and able to work independently.Reliable transportation to travel within a local service area.Why Compass Healthcare?
