JOB DESCRIPTION SUMMARY: The Patient Care Coordinator’s (PCC) primary responsibility is to provide administrative support for the Clinical Managers, clinical care staff and patients. The PCC will coordinate daily/weekly/monthly clinical reports, patient supplies and interagency/community referrals. The PCC will also collaborate with agency departments, facilities and physicians as needed to coordinate appointments and/or obtain necessary documents required for care delivery and billing. The PCC must be clinically licensed & have the ability to receive a physician order. This position is primarily in an office environment; however, the PCC may be asked to assume patient overflow during high census situations.
ESSENTIONAL JOB FUNCTIONS/RESPONSIBILITIES
1. Collaborate with scheduling department to verify employee/patient schedules are accurate, staff are productive & schedule variances are reconciled.
2. Support patient needs by communicating with field staff when patient assignments need priority i.e., clinical change in condition, with direction from Clinical Manager.
3. Supports safe, effective care delivery by assisting with order review to ensure State and Federal Regulations are satisfied.
4. Supports operations & compliance by effective maintenance of agency Clinical Tasks Reports for variances. Position Summary: The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under
the supervision of a registered nurse. Responsibilities include following the plan of care,
providing treatments, and working collaboratively with the members of the team to help meet
positive patient care outcomes.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care:
Provides direct patient care as defined in State’s Name Nurse Practice Act.Implements plan of care initiated by the registered nurse.Provides accurate and timely documentation consistent with the plan of care.Assesses and provides patient and family/caregiver education and information pertinent todiagnosis and plan of careParticipates in coordination of home health services, appropriately reporting the identified needs forother disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or ClinicalManager/Clinical Director.Uses equipment and supplies effectively and efficiently.Participates in personal and professional growth and development.Performs other duties as assigned by the registered nurse.Communication:
Prepares clinical notes and updates the registered nurse when necessary.Communicates effectively and works closely with the office is team in a professional manner andensures they are aware of any scheduling or patient difficultiesInforms manager and/or RN of any patient care concerns or issues.POSITION QUALIFICATIONS
1. Graduate of an accredited practical nurse or vocational nursing program.
2. Has three years nursing experience and, effective January 1, 1998, Shall have one year prior professional nursing
experience in Community/home health or medical/surgical experience.
3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
4. Complies with accepted professional standards and practice.
5. Demonstrates good verbal and written communication, and organization skills.
6. Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is
in good working order.
7. Possesses and maintains current CPR certification.
SKILLS REQUIRED
1. Self-directed and able to work with minimal supervision.
2. Demonstrates excellent observation, problem solving, verbal and written communication skills, nursing skills per
competency checklist.
3. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and
equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform
required nursing skills
