    POSITION PURPOSE

    Adobe Population Health is hiring a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for both pediatric and adult members. This role focuses on improving health literacy and overall quality of life through comprehensive annual visits.

    The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health. Visits may include:

    In-home Well Child visitsIn-home annual adult wellness exams

    The NP will also:

    Assess psychosocial and clinical needs of patients and familiesCoordinate and communicate care plans with primary care and community providersSupport appropriate utilization of healthcare resourcesEnsure timely and accurate documentation across the continuum of care

    This part-time position requires a minimum commitment of 10 hours per week. Availability must include:

    Weekdays: Monday through Friday, 3:00 p.m. – 8:00 p.m.Weekends: Saturday and Sunday, 8:00 a.m. – 5:00 p.m.

    DUTIES & RESPONSIBILITIES

    Travel up to 90% throughout designated regions, which may be updated based on business needs.Conduct in-home assessments on identified patients, following established guidelines.Perform annual in-home Well Child visits for children ranging from 6 months to 21 years of age. Understand the importance of preventative health evaluations focused on monitoring physical, developmental, behavioral, emotional health, and the assessment of social determinants of health (SDOH).Complete In-Home Risk Assessments (IHRA).Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.Identify urgent and emergent situations for proper intervention.Educate members on topics related to health conditions and the continuation of care.Identify diagnoses to be used in care management and active medical management.Provide care coordination to alleviate identified needs related to behavioral, physical, and social issues.Focus on addressing issues that will decrease medical costs and increase the quality of life. Develop positive relationships with clients, caregivers, providers, and other team members.Managing physical health problems through the provision of education.Communicate gaps in care to the member’s primary care physician if needed.Home safety evaluations.Identify internal and external referral needs when available.Compliant with all HIPAA regulations and maintain the security of protected health information (PHI).

    SKILLS & QUALIFICATIONS

    One (1) year of experience as an NP is preferred. Experience in the care of the elderly is preferred.Experience in home health, physician’s office, or public health is preferred.Computer skills required: (Microsoft Office Suite Software).Adapts to changes in policies, procedures, new techniques, & additional responsibilities.Knowledge of HIPAA regulations.Bilingual (English/Spanish) is preferred.

    EDUCATION, LICENSES, & CERTIFICATIONS

    Master's Degree in Nursing is required. Current and unrestricted licensed Family Nurse Practitioner.Current CPR certification.Current BLS certification.Annual TB testing.

    POSITION PURPOSE

    The LVN Community Care Coordinator plays a vital role in delivering comprehensive care coordination and in-home support for patients with complex medical, behavioral, and social needs. This proactive, multi-disciplinary approach ensures improved quality of life, health literacy, and care outcomes while reducing medical costs. The role combines clinical expertise, compassionate patient engagement, and collaboration with a diverse care team to address the holistic needs of patients in the community.

    This position requires travel to San Diego, throughout San Diego County, and surrounding areas to provide in-home support to our patients.

    DUTIES & RESPONSIBILITIES

    Conduct in-home care visits to assess patient conditions and chart observations in the electronic medical record (EMR).Perform vital signs checks, collect medical history, conduct medication reconciliation, and report findings to the patient’s provider.Provide education and support on chronic illness management, safe medication use, and general self-care practices.Deliver palliative care as needed to enhance patient comfort and quality of life.Conduct home safety evaluations and make recommendations for devices or tools to improve patients' daily living.Collaborate with multidisciplinary teams, including physicians, occupational therapists, physical therapists, and social workers, to execute care plans effectively.Engage in community outreach to connect patients with necessary services and resources, assisting with enrollment as needed.Actively participate in bi-weekly panel management and clinical rounds.Perform phlebotomy, venipuncture, and specimen preparation for lab analysis.Provide emotional support, stress management education, and guidance on managing dietary concerns.Document all care coordination activities accurately in the EMR, ensuring compliance with HIPAA regulations.Travel to multiple patient homes or designated locations daily, including shelters, transitional housing, and assisted living facilities.Act as a liaison between patients and healthcare providers, explaining care plans and addressing concerns.Support outreach teams in locating and engaging patients who are difficult to reach.Adapt to changing policies, procedures, and responsibilities as needed.Other duties as assigned.

    SKILLS & QUALIFICATIONS

    Minimum of 1 year of experience as an LVN, preferably in an outpatient, home health, or public health setting.Experience in caring for elderly or chronically ill patients is preferred.Strong organizational, communication, and time-management skills.Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.Ability to work independently while collaborating within a multidisciplinary team.Proficiency in Microsoft Office Suite.Proficiency in electronic health records (EHR) systems.Bilingual in English/Spanish is preferred.Must be able to travel up to 40% across the assigned geography.

    EDUCATION, LICENSES, & CERTIFICATIONS

    Graduate of an accredited nursing program with a valid Licensed Vocational Nurse (LVN) license in California is REQUIRED. CPR/BLS certification (AHA for healthcare providers).Certified phlebotomy skills are highly desirable.Annual TB testing. Unrestricted driver’s license.

    POSITION PURPOSE

    Adobe Population Health is hiring a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for both pediatric and adult members. This role focuses on improving health literacy and overall quality of life through comprehensive annual visits.

    The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health. Visits may include:

    DUTIES & RESPONSIBILITIES

    Travel up to 90% throughout designated regions, which may be updated based on business needs.Conduct in-home assessments on identified patients, following established guidelines.Perform annual in-home Well Child visits for children ranging from 6 months to 21 years of age. Understand the importance of preventative health evaluations focused on monitoring physical, developmental, behavioral, emotional health, and the assessment of social determinants of health (SDOH).Complete In-Home Risk Assessments (IHRA).Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.Identify urgent and emergent situations for proper intervention.Educate members on topics related to health conditions and the continuation of care.Identify diagnoses to be used in care management and active medical management.Provide care coordination to alleviate identified needs related to behavioral, physical, and social issues.Focus on addressing issues that will decrease medical costs and increase the quality of life.     Care Coordination Compliance: Ensure care coordination complies with 42 C.F.R. § 438.208 and all relevant agreement requirements.Health Risk Assessment: Perform standardized Health Risk Assessments (HRAs) as per New Mexico Health Services Department guidelines, determining if members need comprehensive assessments.Member Classification: Accurately place each member in appropriate care coordination levels as per agreement standards.Comprehensive Needs Assessment (CNA): Conduct CNAs for members meeting specific conditions, complete related questionnaires, and agreements, and inform members about community benefits tailored to their needs.Care Plan Development: Develop and implement Comprehensive Care Plans (CCPs) based on individual member needs and preferences in alignment with the agreement guidelines.Ongoing Care Delivery: Deliver continuous care coordination services in accordance with the member’s assessed needs, CCP, and required frequency of contact.Response to Member Needs: Consistently assess and respond to members' needs for services and assistance.Level 2 and 3 Coordination: Conduct CNAs for members meeting specified indicators to determine their assignment to care coordination level 2 or 3, and ensure compliance with requirements for members not assigned to these levels.Team Collaboration: Utilize a care coordination team approach, collaborating with various health and community resources to address member needs effectively.Other Duties as assigned by management.

    SKILLS & QUALIFICATIONS

    Two (2) years of relevant of social work or healthcare-related experience strongly preferred.     Conduct in-home care visits to assess patient conditions and chart observations in the electronic medical record (EMR).Perform vital signs checks, collect medical history, conduct medication reconciliation, and report findings to the patient’s provider.Provide education and support on chronic illness management, safe medication use, and general self-care practices.Deliver palliative care as needed to enhance patient comfort and quality of life.Conduct home safety evaluations and make recommendations for devices or tools to improve patients' daily living.Collaborate with multidisciplinary teams, including physicians, occupational therapists, physical therapists, and social workers, to execute care plans effectively.Engage in community outreach to connect patients with necessary services and resources, assisting with enrollment as needed.Actively participate in bi-weekly panel management and clinical rounds.Perform phlebotomy, venipuncture, and specimen preparation for lab analysis.Provide emotional support, stress management education, and guidance on managing dietary concerns.Document all care coordination activities accurately in the EMR, ensuring compliance with HIPAA regulations.Travel to multiple patient homes or designated locations daily, including shelters, transitional housing, and assisted living facilities.Act as a liaison between patients and healthcare providers, explaining care plans and addressing concerns.Support outreach teams in locating and engaging patients who are difficult to reach.Adapt to changing policies, procedures, and responsibilities as needed.Other duties as assigned.

    SKILLS & QUALIFICATIONS

    DUTIES & RESPONSIBILITIES

    Conduct in-home care visits to assess patient conditions and chart observations in the electronic medical record (EMR).Perform vital signs checks, collect medical history, conduct medication reconciliation, and report findings to the patient’s provider.Provide education and support on chronic illness management, safe medication use, and general self-care practices.Deliver palliative care as needed to enhance patient comfort and quality of life.Conduct home safety evaluations and make recommendations for devices or tools to improve patients' daily living.Collaborate with multidisciplinary teams, including physicians, occupational therapists, physical therapists, and social workers, to execute care plans effectively.Engage in community outreach to connect patients with necessary services and resources, assisting with enrollment as needed.Actively participate in bi-weekly panel management and clinical rounds.Perform phlebotomy, venipuncture, and specimen preparation for lab analysis.Provide emotional support, stress management education, and guidance on managing dietary concerns.Document all care coordination activities accurately in the EMR, ensuring compliance with HIPAA regulations.Travel to multiple patient homes or designated locations daily, including shelters, transitional housing, and assisted living facilities.Act as a liaison between patients and healthcare providers, explaining care plans and addressing concerns.Support outreach teams in locating and engaging patients who are difficult to reach.Adapt to changing policies, procedures, and responsibilities as needed.Other duties as assigned.

    SKILLS & QUALIFICATIONS

    Minimum of 1 year of experience as an LVN, preferably in an outpatient, home health, or public health setting.Experience in caring for elderly or chronically ill patients is preferred.Strong organizational, communication, and time-management skills.Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.Ability to work independently while collaborating within a multidisciplinary team.Proficiency in Microsoft Office Suite.Proficiency in electronic health records (EHR) systems.Bilingual in English/Spanish is preferred.Must be able to travel up to 40% across the assigned geography.

    POSITION PURPOSE


    The Community Care Coordinator is responsible for providing comprehensive care coordination and management services to clients within an assigned territory.


    This role combines field-based care coordination with supervisory responsibilities, focusing on enhancing client outcomes through behavioral, physical, and social interventions.     This position requires travel to Flagstaff, around Coconino County, and surrounding areas to provide in-home support to our members.

    PRN EMPLOYMENT STATUS

    This is a Per Diem (PRN) position and is scheduled on an “as-needed” basis to support operational staffing needs. PRN employees may be utilized to cover planned and unplanned absences including call-offs, vacations, extended leave, and vacant positions. Adobe Population Health is not obligated to offer shifts, and PRN employees are not guaranteed consistent scheduling beyond program participation requirements.

    DUTIES & RESPONSIBILITIES

    Resource Sharing and Care Coordination

    Provide field-based care coordination to address behavioral, physical, and social needs of assigned clients, aiming to reduce medical costs and improve quality of life. Assess the social, financial, and health needs of individuals and families within the community.Connect clients with appropriate services, including housing assistance, food security programs, healthcare services, transportation, and mental health resources.Develop and implement individualized care plans in collaboration with clients, ensuring that their unique needs are addressed.Provide ongoing support to ensure clients' needs are met, including follow-ups and reassessments.Advocate on behalf of clients to ensure they receive timely access to services and resources.

    Health Education and Support

    Offer education on physical health, illnesses, treatments, dietary concerns, stress management, and safe medicine use. Provide emotional support to clients and manage their overall well-being.  Support clients in developing self-care strategies and accessing preventive health services.Encourage clients to actively engage in their health and well-being through empowerment and education.

    Community Outreach and Engagement

    Conduct home safety evaluations and monitor client progress through regular home visits. Connect clients with appropriate community services and resources.Build and maintain strong relationships with local community organizations, healthcare providers, and government agencies to ensure access to a wide range of resources.Participate in outreach efforts to identify individuals and families in need, particularly in underserved or marginalized communities.Represent the organization at community events, health fairs, and workshops to promote services and educate the public about available resources.Identify trends and emerging needs within the community to inform organizational planning and service delivery.

    Data Collection and Record Keeping

    Maintain accurate records of care coordination in the Electronic Medical Record (EMR) system and ensure compliance with HIPAA regulations.  Document interactions, care plans, and outcomes in accordance with organizational policies and confidentiality guidelines.Track and report on the effectiveness of referrals and interventions to help identify gaps in services or areas for improvement. Maintain accurate records of client information and provide timely reports to supervisors and funding agencies as required.Participate in program evaluation and quality improvement initiatives.

    Policy and Procedure Compliance

    Ensure adherence to Adobe policies and procedures, and assist in developing and updating company programs, policies, and procedures as needed. 

    Relationship Building

    Develop and maintain positive relationships with case managers, providers, administration, and other stakeholders. 

    Travel and Meetings

    Travel to assigned territories for training, meetings, and as requested by upper management. Attend all applicable facility and management meetings.

    PRN Coverage Expectations

    Maintain availability for a minimum of one (1) eight-hour shift per week. Work a minimum of four (4) shifts per month to remain active in the PRN Staffing Program. Respond and assist with staffing requests at least three (3) out of every four (4) times contacted when reasonably available. Report to assigned shifts at scheduled start times or within two (2) hours when covering unplanned absences. Accept assignments within designated service areas as directed by leadership. Notify management in advance of any known periods of unavailability following company policy.

    SKILLS & QUALIFICATIONS

    Four to six years of related experience and/or training, or an equivalent combination of education and experience. Medical experience strongly preferred.Experience working with diverse populations, including marginalized, low-income, and underserved communities.Must have the willingness to drive with a clean driving record.Reliable transportation.Clean driving record.     Associate’s degree in Social Work, Sociology, Psychology, Counseling, or a related field required, OR active LPN/RN licensure, EMT, or Paramedic certification; Bachelor’s degree preferred. Current BLS certification and annual TB testing required.  Current and clear driver’s license.

    COMPENSATION & BENEFITS (PRN)

    PRN employees are compensated at a premium hourly rate and are not eligible for benefits including but not limited to paid time off, holiday pay, bereavement leave, medical, dental, vision, life insurance, or disability insurance. Workers’ Compensation coverage is provided in accordance with state and federal regulations. PRN employees are not eligible for annual salary adjustments.
