    POSITION TITLE: Licensed Practical Nurse

    DEPARTMENT: Medical/Nursing

    REPORTS TO: Nursing Director

    ENTRY LEVEL QUALIFICATIONS:

    Graduate of approved School of Nursing, current State license (RN-preferred) (LPN acceptable), two years’ experience in chemical dependency treatment preferred. Experience with adults and adolescents preferred.

    1. Must be able to continuously sit from ½ to 4 hours per 8-hour shift.

    2. Must be able to see 20/40 corrected in each eye and ear (hearing aid acceptable).

    3. Must be capable of administering First Aid and CPR.

    4. Walk within the facility approximately 1 mile per 8-hour shift.

    5. Must be emotionally and physically capable of functioning in stressful situations.

    6. If recovering, must have two (2) years continuous abstinence.

    GENERAL RESPONSIBILITIES:

    Provide all duties of nursing services. Follow Nursing policy and procedures regarding detoxification and other Nursing Services. Carry out physician’s orders and protocols as directed. Participate in Performance Improvement and Infection Control activities as indicated.

    Know and abide by the general provisions of 42 CFR Part 2, “Confidentiality of Drug and Alcohol Abuse Patient Records”, and 45 CFR “Health Insurance Portability and Accountability Act.” This position has unrestricted access to patient identifying and patient health information.

    KEY FUNCTIONS:

    Complete and document an accurate and comprehensive Nursing Assessment within twenty-four (24) hours of patient’s admission.Assess acuity of withdrawal and intervene according to Physician’s policy, procedures, and protocol.Establish and maintain rapport with patients and their family members.Monitor vital signs (B/P, Pulse, Respirations) and administer medications when ordered by physician.Perform venipuncture on patients without harm.Administer P.O. medications, IM injections and IV medications, including IV fluids.Transcribe physician orders, order medication and be accountable for controlled medications.Perform routine monitoring and testing, e.g., blood glucose monitoring, audiometric screening, EKG.Provide health/medical education to patients and family members as appropriate.Participate in timely, accurate patient record documentation pertaining to nursing assessments, treatment planning, treatment rendered, patient’s response to and progress in treatment, physicians’ orders, to include “repeat” orders, and other standard documentation activities.Participate in Clinical Team Staffing and staff meetings as appropriate.Conduct regular performance improvement monitoring and evaluation activities and serve on Task Groups as needed.Practice the Board of Nursing Standards of Practice.Maintain an environment conducive to well-being, safety, and provide health teaching and counseling to patients and staff.Must maintain a professional demeanor and appearance, must not appear impaired or disoriented, but rather must present coherent and articulate speech, kept appearance, alert, oriented, and responsive to patients' needs and concerns.Regular attendance.Must be able to work in a cooperative manner with co-workers, managers, clients, and prospective clients.Must be able to work in a constant state of alertness to perform the job in a safe manner.Perform urine drug screens as necessary.Other duties as assigned.

    2.Maintain an "UR Activity Log" which records every review for the facility with financial classification and U.R. information/activity.
    3.Maintain a "call back' calendar for continued stay reviews and maintain a U.R. file for each patient.
    4.Document, in the patient electronic record software, the review with the managed care organization, including the level of care requested, the status of the request, outcome and information about the next review.
    5.Review and maintain all correspondence pertaining to certification, appeals, denials or reconsiderations, and ensure they are scanned into the patient electronic health record.
    6.Complete all Quality Improvement indicators for Utilization Management, data analysis and reporting.
    7.Educate counseling staff regarding data needed to adequately document patient care and educate crisis staff regarding data needed to adequately document intake assessments and managed care criteria.
    8.Ability to work in a constant state of alertness as to perform the job safely.
    9.Ability to work in cooperative manner with co-workers, managers, clients and prospective managers.

    3. Must be able to continuously sit for approximately three (3) hours per eight (8) hour shift.

    This position will interact directly with potential clients and/or their family members as well as new and existing referral sources and other internal Bradford stakeholders to facilitate the pre-admission process. The goal of these interactions is to collect relevant information regarding the impact of chemical dependency and/or substance use disorder on the lives of potential patients and to motivate those individuals into taking appropriate action regarding suggested treatment.

    II. PRIMARY DUTIES AND RESPONSIBILITIES:

    Create a strong first impression of Bradford Health Services to prospective clients, family members and referral sources by answering inbound calls, building rapport with those individuals, and gathering the initial information needed to capture them in our CRM system. Complete pre-admissions screenings with prospective clients and compare against established intake criteria, policies, and procedures to move patients forward in the pre-admission process where possible and to escalate for supervisor review where necessary.Where appropriate, speak with referral sources, family members and other patient stakeholders to gather relevant collateral information.Evaluate patient insurance and financial eligibility, and discuss financial terms with clients and family members.Use all information collected to motivate the patient into a decision-making process, helping them to understand the suggested level of care, and persuading them to start treatment as soon as possible.Advocate for potential clients both internally and externally to clear any roadblocks to admission.Appropriately collect, record, and convey pertinent client information to supervisors, clinical and medical teams, and other staff, as necessary.Provide referrals to patients where necessary, connecting them with appropriate resources in their areas.Send appropriate follow up materials - included screening summary letters, drug screens, etc – to referral sources where warranted.Follow HIPAA and 42 C.F.R. Part 2 regulations to always ensure the protection of patient health information.Track all potential patients not admitted to the appropriate level of care via and follow up where appropriate in an effort to convert to admission.Complete all assigned on ongoing trainings and competencies.Record daily all patient-related activities in the Customer Relationship Management system using clear, concise, accurate documentation.

    III. ESSENTIAL JOB REQUIREMENTS:

    A. Education and Experience:

    Bachelor's Degree and experience or working knowledge of addiction and treatment is preferred CRSS certification a plus.Knowledge of chemical dependency preferred.Customer service and/or sales experienced required.Computer data entry skills required – type 40 wpm required. Customer Relationship Management software experience preferred.

    B. Knowledge, Skills and Abilities:

    Maintain appropriate professional boundaries and confidentiality, including a working knowledge of 45 C.F.R. “Health Insurance Portability and Accountability Act” and 42 C.F.R.     A. Education and Experience:


    • Bachelor’s degree in business, social science, or human resources required, master’s degree preferred.

    • Minimum of five years’ experience in human resources or related field.

    10. Conduct group therapy sessions appropriate for the treatment of addictive disorders.
