    Registered Nurse (RN) in the areas of:

    JOB SUMMARY:

    A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager/Nursing Supervisor.

    QUALIFICATIONS:

    Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.One (1) year nursing experience, preferred.Acceptance of philosophy and goals of this Agency.Ability to exercise initiative and independent judgment.

    LINE OF AUTHORITY: Reports to the Clinical Manager/Nursing Supervisor

    RESPONSIBILITIES:

    Provides services in accordance with the plan of care.Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.Initiates the plan of care and necessary revisions.Furnishes those services requiring substantial specialized nursing skill.Initiates appropriate preventive and rehabilitative nursing procedures.Prepares clinical and progress notes for each patient visit and summaries of care conferences on patients in a timely manner as per Agency policy.Coordinates services.Informs physician and other personnel of changes in the patient’s condition and needs.Counsels the patient and family/significant others in meeting nursing and related needs.Participates in in-service programs, supervising and teaching other nursing personnel.Understands and adheres to established Agency policies and procedures.Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.Determines the amount and type of nursing needed by each individual patient.Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.Supervises the LPN/LVN every 30-days and supervises the HHA/CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel.Conducts patient care conferences on patients assigned to his/her care.Participates in peer review and Quality Assessment Performance Improvementas assigned.Participates in utilization review of medical records as assigned.Gives total patient care as needed.Takes on-call duty nights, weekends and holidays, as assigned.Completes and submits OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.Appropriately utilizes ICD-10 diagnoses and codes.     Job Title: Home Health Clinical Quality Assurance Specialist (OASIS & Medicare Compliance)

    Applicants must have OASIS experience in home health care setting.

    Position Overview:
    The Home Health Quality Assurance Specialist ensures the accuracy, compliance, and quality of patient care documentation, with a primary focus on OASIS (Outcome and Assessment Information Set) data. This position involves reviewing patient records, conducting quality assurance checks, and exporting OASIS items to Medicare for compliance and reimbursement purposes. The specialist collaborates with clinical staff to ensure documentation meets regulatory standards and helps optimize patient outcomes and agency performance.

    Key Responsibilities:


    1. OASIS Data Review & Export:
    • Review and validate OASIS assessments for accuracy and completeness before submission.
    • Export OASIS data to Medicare ensuring compliance with CMS (Centers for Medicare & Medicaid Services) regulations.
    • Address any discrepancies or errors in OASIS data and collaborate with clinicians to correct inaccuracies.
    2. Quality Assurance & Compliance:
    • Conduct regular audits of clinical documentation to ensure adherence to state, federal, and agency regulations.
    • Monitor compliance with Medicare guidelines, particularly around OASIS and other key reporting metrics.
    • Implement quality improvement initiatives to enhance documentation practices and patient care outcomes.
    3. Training & Support:
    • Provide training to clinical staff on accurate OASIS data entry and Medicare compliance.
    • Stay updated on changes in Medicare regulations and OASIS requirements and communicate updates to relevant staff.
    4. Collaboration:
    • Work closely with the clinical team to ensure patient assessments are properly documented and reflect the care provided.
    • Collaborate with billing and reimbursement teams to ensure timely and accurate submissions to Medicare.
    5. Reporting & Analytics:
    • Generate reports on OASIS data submissions and quality assurance activities.
    • Analyze trends in documentation errors or compliance issues and recommend strategies for improvement.

    Qualifications:
    . Experience or certification in QA and oasis management.
    • Strong knowledge of OASIS, Medicare, and CMS regulations.
    • Experience in quality assurance, auditing, or compliance within healthcare.

    RESPONSIBILITIES:

    Performs services planned, delegated and supervised by the Physical Therapist.Assists in preparing clinical and progress notes.Participates in educating the patient and family.Participates in in-service programs and presents in-service programs as assigned.Understands and adheres to established Agency policies and procedures.Provides physician prescribed physical therapy under a written plan of care established by the PT.Participates with all other health care personnel in patient care planning.Consults with PT regarding change in treatment.Instructs patients and family members in home programs and activities of daily living.Attends all staff meetings and patient care conferences as scheduled.Prepares medical records, progress notes and updates care plans for each patient visit in a timely manner as per Agency policy.Informs the PT and other team members of changes in the patient's condition and needs.Continually reevaluates needs of patient/family.Coordinates the therapy aspects of patient care with the PT.Consults with PT to determine whether treatment plan should be continued, changed or terminated.Instruction to patient/family in home program with documentation in medical record notes.Observes confidentiality and safeguards all patient related information.Completes and submits progress notes and paperwork in a timely manner per Agency policy.Immediately reports to supervising Physical Therapist any patient incidents/variances or complaints.Demonstrates competent performances of technical skills according to established procedures.Participates in peer review and QAPI activities as requested.Adheres to Agency standards and consistently interprets and accurately performs all assigned responsibilities.Maintains acceptable attendance status, per Agency policy.Reports all incomplete work assignments to supervising Physical Therapist.Appearance is always within Agency standards; is clean and well groomed.Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments.Maintains clean and neat work environment.Demonstrates sound judgment and decision making.Maintains current CPR certification.Performs other duties as assigned.

    WORKING ENVIRONMENT:

    Works indoors in Agency office and patient homes and travels to/from patient homes.

    JOB SUMMARY:

    A paraprofessional who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN).

    QUALIFICATIONS:

    RESPONSIBILITIES:

    Provides services that are ordered in the plan of care and that the aide is permitted to perform under State law.Performs hands-on personal care.Performs simple procedures as an extension of the therapy or nursing services as assigned.Assistance in ambulation or exercises as assigned.Assists with medications that are ordinarily self-administered as assigned.Understands and adheres to established Agency policies and procedures.Completes appropriate visit records in a timely manner as per Agency policy.Reports changes in the patient's condition and needs to the appropriate personnel.Performs household services essential to health care in the home as assigned.Attends in-service and continuing education programs as scheduled and necessary.Attends staff meetings and patient care conferences as scheduled.Follows Agency policy for cleaning equipment between patient uses.Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.Completes all work assigned by the RN.Does not accept assignments for a patient with special needs for which he/she has not received appropriate training.Observes confidentiality and safeguards all patient related information.Reports any variance, accident or unusual occurrence to the appropriate personnel.Participates in QAPI activities as requested.Maintains acceptable attendance status, per Agency policy.Reports incomplete work assignments to RN.Appearance is always within Agency standard; is clean and well groomed.Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments.Attends position related in-services.
