    SUMMARY

    Provide on-going training and personal assistance to Community Habilitation participants with disabilities with the intent of promoting independence and community inclusion. Work with minimal supervision, provide training, role modeling, structured routines and specialized assistance as outlined in each participant’s habilitation plan. As much as possible, routines will be individualized, based on the participant’s choice of activities and aimed at empowerment and inclusion in the community.


    ESSENTIAL DUTIES AND RESPONSIBILITIES

    The duties and responsibilities of the Community Habilitation Specialist will include but are not limited to the following:

    Adhere to ethical principles including the agency’s mission statement and the standards of conduct.Provide support to participant in choosing and participating in a variety of meaningful recreational, vocational, and daily living skills activities based on participant desires, parameters of the program, and their individualized community habilitation plan.Assess the interests, abilities, strengths and limitations of the people we support.Follow dietary guidelines as directed in habilitation plan.Provide personal care consistent with participants skill level and as outlined in the habilitation plan (including bathing, grooming, toileting, dressing, laundry, etc.).Demonstrate sensitivity and responsiveness to the people we support.Work with supervisors and co-workers to meet program needs.Develop an appropriate rapport and work effectively with the circle of support of the people we support.Keep written and verbal information shared with team members regarding a participant confidential in accordance with HIPPA regulations.Work with other service providers within the community in a professional manner.Participate in regular staff meetings and required trainings.Carry out responsibilities as assigned by the program manager (i.e., doctor’s appointments, religious observances, banking, budgeting, session notes, goal plans, etc.).Utilize pro-active approach to avert crisis situations.Utilize on-call supervisor to receive instruction and report emergency situations.Provide opportunity and advocacy in the areas of continuing education, technology access, communication systems and emotional well-being.Research community resources and provide linkage to increase inclusion within the community of the people we support.Foster independence and autonomy.Other duties as assigned.

    QUALIFICATIONS

    Ability to read, write and speak English.Computer literacy at a level that will accommodate training in and utilization of Electronic Medical Record software.Ability to implement a prompting hierarchy and write observant and detailed notes for each session performed by employee.Flexible schedule to meet participants needs.Willing to travel to the homes of the people we support or meet them in the community.Successful completion of required in-service training.Must participate and meet established requirements in a date base check through the Division of Criminal Justice Services as defined by the State of New York, Office of People with Developmental Disabilities, and Chapter 575 of the Laws of 2004.Must participate in successful database checks through the State Central Registry of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.


    EDUCATION and/or EXPERIENCE

    High school diploma required.Some college and experience working with people with disabilities preferred.


    COMPENSATION: $19.75 - $20.81/hour (after probation) + Industry-Leading Benefits for all full-time employees. We are offering sign-on bonus of $1,500 to new employees

    SUMMARY

    Under general supervision, the Investigator supports the integrity and safety of ADAPT Community Network’s programs by conducting thorough internal investigations, providing administrative oversight of incident reporting systems, and ensuring compliance with OPWDD and Justice Center regulations. This role also delivers training to staff, interfaces with regulatory agencies, and contributes to data tracking and trend analysis.


    ESSENTIAL DUTIES AND RESPONSIBILITIES

    The duties and responsibilities of the Investigator will include, but are not limited to the following:

    Conducting agency investigations, as assigned.Provide administrative support to the Program Support Department.Provides training to staff regarding conducting investigations, incident reporting, and related topics.Assist with management and tracking of information in OPWDD’s IRMA database and interface with the Justice Center to ensure incident closure.Coordinates completion of annual OPWDD Utilization Reviews for ICF Programs.Assists in completion of required Justice Center target letters, submission of SCR target searches, and submission forms for Investigative Records for Abuse and Neglect Allegations.Interfaces with OPWDD incident coordinators to ensure proper classification of incidents.Assists in preparation of annual trend information for incidents.


    QUALIFICATIONS

    SUMMARY

    Under general direction, is responsible for conducting educational classes for special education pre-school or Early Intervention, in accordance with the needs of the students and in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies. Acts as team leader of each student’s team. The Teacher is a management position and supervisory position. Is responsible for supervising Teacher Assistants assigned to his/her team. Interviews, effectively recommends hire, discipline termination, and suspension; resolving grievances, schedules, orient; responsibly directs; completes probationary and annual evaluations; and commends Teacher Assistants.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    The duties and responsibilities of the Special Education Teacher will include, but are not limited to the following:

    Plans and conducts classes/activities adapted to needs of students.Assesses and prepares IFSP/IEP’s. Implements and evaluates short and long-term goals and objectives.Participates in activities pertaining to student transitioning.Participates in Initial Evaluations and Intake processes.Provides educational assessment and evaluation on students annually in all educational domains.Prepares and adapts all individual and group lesson plans and materials, equipment and furniture arrangement and necessary modifications of environment.Maintains records of daily attendance and progress. Writes periodic progress reports, annual reports and other data as requested.Ensures that services are rendered to students with utmost regard for their personal dignity, privacy and confidentiality.Participates in activities outside classroom where necessary, i.e. trips, pool, gym, libraries, lab, etc.Serves and participates as a member at interdisciplinary team meetings, Annual Reviews, Parent Meetings, Special Review Committee meetings, In-Services, etc. as required.Attends regular staff meetings, student reviews, district meetings and conferences.Orients, trains, evaluates, delegates, supervises work of and effectively recommends hiring, disciplining, suspending and terminating of Teacher Assistants assigned to his/her team.Obtains approval, requisitions and purchases class equipment and materials.Assists with transporting students between buses and building as needed.Implements universal precautions at all times.Assists with toileting, feeding and positioning students as needed.Attends supervisory and management meetings.Assists students on arrival and departure with their outer clothing as needed.Substitutes in another classroom as requested.Participates in in-service training programs as requested.Confers with director or other agency personnel regarding problems, questions and suggestions for programming or handling of students.Initiates precautions within established framework to assure that own work is completed in a manner free from recognized hazards.Attends parent meetings as requested. Confers with individual parents on program content and progress as it relates to their child.Assists with curriculum development.Provides supervision and training to student teachers.Keeps abreast of changes in field.Performs other related duties as requested.

    SUMMARY

    Under general supervision, is responsible for devoting their attention to the care and development of the people we support in one of the Agency’s residential facilities. Interacts in a manner which fosters dignity and learning in a way that helps lead to greater independence. Helps create a homelike atmosphere for the people we support in an agency facility.


    ESSENTIAL DUTIES AND RESPONSIBILITIES

    The duties and responsibilities of the Residence Program Specialist II will include but are not limited to the following:

    Provides care and assists with personal development and independence of the people we support.Ensures that all measures for safety and well-being are maintained at highest level.Provides training for the people we support in activities of daily living and in development of self-help and social skills.Maintains a personalized, homelike atmosphere in the residential facility.Attends weekly staff meetings as well as any other assigned meetings and in-service trainings.Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned.Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action.Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures.Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices.Summons police, fire, ambulance or other emergency personnel if situation warrants it.If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines.Assists with transportation for the people we support to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc.Assists with meal planning and preparation as well as with purchasing of food and supplies.Demonstrates an understanding of and implements behavioral programs.Participates fully as a member of interdisciplinary plan.Maintains an ongoing informal exchange of information. Maintains written logs for recording daily activities.Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment.Supervises any other activities as may be necessary to maintain quality of the program.Dispenses prescribed medications to the people we support after successful completion of the medication administration course, and in line with Agency protocol. AMAP’s are supervised by residential nurse in all areas related to medication administration.Completes all activities related to skill buildings as outlined in individual program plansProvides support around personal hygiene, i.e. toileting, bathing and changing as needed.Handles all personal documents of the people we support in a secure and confidential manner.Performs related duties as requested.

    SUMMARY

    Under general supervision, is responsible for providing medical treatment, first aid, emergency care and medications, under the direction of a Registered Nurse and in accordance with ADAPT Community Network policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession.


    ESSENTIAL DUTIES AND RESPONSIBILITIES

    The duties and responsibilities of the Licensed Practical Nurse - Residential will include but are not limited to the following:

    Serves as a member of interdisciplinary team and is responsible for assisting Registered Nurse in coordinating following through on provisions of health care services for assigned residences.Keeps progress notes under supervision of registered nurse for each client and summarizes for quarterly, semi-annual and annual reviews.Provides health care to individuals receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to individuals receiving services and families as required.Handles control of communicable diseases and infections through identification and assessment, reporting any finding to registered nurse and consulting physician, implementing appropriate protective and preventive measures and monitoring infection control practices.Provides first aid to persons in need of immediate care, with referral to registered nurse or outside emergency services as necessary.Assists physicians in medical examinations as needed.Maintains a tickler for all appointments. Schedules appointments and follows up on physicians’ recommendations.Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes.Administers medications when appropriate. Supervises storage of medications and assists in oversight and ordering of medication.Assists registered nurse in maintaining AMAP re-certification files.Obtains consent for surgery of residents in absence of registered nurse.Refers all consults and diagnostic reports to registered nurse for review.Attends and transports clients to appointments as necessary.Attends team meetings as scheduled.Attends to clients’ medical conditions.Carries beeper and is on-call, responds to questions regarding physician and RN treatment plans.Keeps abreast of changes in Nursing field.Performs other related duties as requested.

    SUMMARY

    SUMMARY

    Under general supervision, is responsible for the development and implementation of the Individualized Service Plans for the people we support and their advocates in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies within a residential setting. Provides case management services to assigned individuals and determines individual aptitudes, interests and preferences. Identifies appropriate service providers and links individuals with providers. Coordinates services and monitors the satisfaction of the people we support with the services received.


    ESSENTIAL DUTIES AND RESPONSIBILITIES

    The duties and responsibilities of the Case Manager will include, but are not limited to the following:

    Provides social services to each person we support working toward maximizing their social functioning, enhancing coping capacity of their family, asserting and safeguarding their human and civil rights, and fostering their human dignity and personal worth.Develops and monitors individual treatment plan in accordance with regulatory and agency policy.Participates in interdisciplinary planning. Interprets, documents, and follows up on all recommendations of interdisciplinary team meetings. Chairs all meetings of the interdisciplinary team.Includes channels of communications and consultation with residential staff and professional staff to identify the needs as the people we support progress through habilitation.Develops goals and activities generated by interdisciplinary team. Produces an active treatment plan schedule for each person served.Documents comprehensive functional assessment and review of plan of care as needed.Ensures proper data collection through observation and in-service training.Assists in all activities of daily living and participates in activities outside of the residence, i.e. shopping, trips swimming pool as needed.Supervises Residence Program Specialists in implementation of active treatment plans.Makes regular and meaningful family, residential, and agency-wide contacts.Trains and instructs Residence Program Specialists on the specific goal plans for the people we support, relevant data collection, daily routine, and general procedures and practices of the residence.Maintains contacts with government social services agencies, not-for-profit social service agencies to access resources and/or advocate for services.Performs individual evaluations to determine whether and when alternate placement is needed.Handles family outreach and in-service training.Ensures that all benefits for which the people we support may be eligible for are applied for in an expeditious manner and to effect transfer of any prior accumulated funds to agency.Provides systematic follow-up to assure referral to appropriate community agencies upon discharge.Works regularly with, develops, or is responsible for maintaining security of confidential information such as records of person receiving services which requires regular use of discretion. Improper disclosure may cause embarrassment, disruption, or conflict to agency or persons receiving services and their families.Coordinates admissions to program including preparation of referral material, initial screening of applicants, contact with families, and other referral services.Ensures all required adaptive equipment is secured and maintained in good working order.Maintains up to date files on applicants.Processes new admissions including contact with families and arrangements for day programming.Advocates for the people we support to ensure that all their rights are being fulfilled.Writes monthly case notes on all interpersonal, social, workshop progress and family contacts of the people we support.Serves as a liaison to Day Programs, Service Coordination, and Clinics as needed.Keeps abreast of changes in field.Performs other related duties as requested.
