Position Summary Work in collaboration with a skilled care team that includes dedicated providers, benefits specialists, front desk receptionists, behavioral health consultants and others to provide quality health services and resources to meet our patients' needs. Assist providers with onsite procedures. Take vital signs, perform phlebotomy and administer medications/vaccines directly to patients.     Position Summary The Maintenance Mechanic performs preventive, routine, and emergency maintenance services to ensure ACCESS facilities are safe, functional, and compliant with regulatory and safety standards. The role supports building systems, life safety infrastructure, and day-to-day facility operations across multiple health center locations. Core Job Responsibilities Perform preventive and corrective maintenance across ACCESS facilities, including general carpentry, minor plumbing, HVAC support, and basic electrical work. Maintain accurate preventive maintenance logs and required documentation to support inspections, audits, and regulatory surveys. Support fire, life safety, and emergency preparedness activities, including safety inspections, emergency drills, and required training. Respond to facility-related emergencies and participate in Incident Command operations as assigned. Repair, install, or modify facility components such as doors, flooring, ceiling tiles, wall surfaces, shelving, cabinetry, and partitions. Perform minor HVAC maintenance, including filter changes, belt replacements, and basic troubleshooting and repairs. Utilize the computerized maintenance management system (CMMS) to receive, track, prioritize, and complete work orders within established service standards. Coordinate with external vendors and contractors to ensure safe, timely, and high-quality maintenance and repair services. Assist with facility moves, furniture and equipment assembly, special event setup, and limited groundskeeping or snow removal as needed. Maintain safe, organized, and secure control of tools, equipment, materials, inventory, and assigned vehicles or storage areas. Other duties as assigned. Requirements/Preferences High School Diploma or equivalent required. At least two years of experience in facility or building maintenance preferred. Experience working in a healthcare, clinical, or regulated environment preferred. Valid driver's license with acceptable driving record. Proof of insurance is required if a personal vehicle is used for work. Working knowledge of general maintenance trades including carpentry, plumbing, HVAC, and basic electrical work. Knowledge of OSHA regulations, safety practices, and applicable building codes. Trade-related certifications (HVAC, carpentry, electrical, locksmith, refrigeration) preferred. Basic computer skills, including email and electronic work order systems. Must be able to lift up to 50 pounds, climb ladders, and perform sustained physical labor. Working Conditions/Equipment Duties are performed mainly in facility and health center settings, with occasional office work. Local travel between health centers and community locations is required. Work schedule generally between 6:00 AM - 2:30 PM, with flexibility for holidays, weekends, and off-hour emergency response as needed. Ability to stand, walk, bend, climb, lift, and perform physical work for extended periods. Work performed indoors and outdoors in varying environmental conditions, including temperatures below 32°F and above 90°F. Regular use of computers, mobile devices, general office equipment, and maintenance tools. Employees are expected to have own required hand tools necessary to perform assigned maintenance duties, as applicable. Read Less

Position Summary: The Patient Navigator plays a critical role in supporting patients by developing and implementing individualized, multi-session, time-limited interventions to assist those with recent diagnoses or positive test results. This position focuses on connecting patients to essential social support services and primary medical care while providing personalized, condition-specific counseling and referrals in a safe and empowering manner. Core Job Responsibilities: Assist patients in navigating the healthcare system, including scheduling appointments, completing paperwork, and explaining treatment plans. Organize and manage appointments with healthcare providers and professionals to ensure timely and comprehensive care. Collaborate with providers, care coordinators, and nurse case managers to enhance patient adherence to healthcare recommendations. Connect patients to community resources, such as medical care, medication assistance, and other healthcare services, and ensure these services are accessed. Establish and maintain a strong working relationship with ACCESS Care Team members to support seamless patient care. Accurately document patient encounters and preliminary eligibility information in organizational and program-specific data systems. Conduct home and community visits face-to-face in accordance with established policies and workflows. Assist with special programs and events to support organizational objectives. Maintain accurate and comprehensive participant records, tracking all contacts and service connections. Perform additional duties as assigned to support organizational goals and priorities. Requirements/Preferences: High School Diploma or GED required. Associate's degree or higher is strongly preferred. Minimum of two (2) years' experience in human services required. Experience in infectious disease care and support services, outreach, health promotion, or related fields is preferred. Demonstrated ability to assess diverse psychosocial needs of clients and patients. Basic proficiency with Electronic Health Record (EHR) systems and EPIC preferred. Bilingual English/Spanish a plus. Intermediate proficiency in Microsoft Office Suite (Word, Excel, PowerPoint). Working Conditions/Equipment Work primarily in a standard office setting, with occasional responsibilities in a health center environment. Regular remote work options are available, requiring a secure, private environment with reliable internet access. Local travel to health centers, hospitals, and community events is required. Ability to sit up to four (4) hours at a time and stand for up to two (2) hours at a time. A reliable mode of transportation required. If using a personal vehicle, a valid driver's license and proof of insurance are mandatory.
