    Conduct thorough patient evaluations: Assess mobility, functional limitations, safety risks, and therapy needs to establish a clear roadmap for successful in-home rehabilitation.Create personalized treatment plans: Design evidence-based, goal-driven therapy plans tailored to each patient's condition, supporting meaningful gains in strength, independence, and confidence.Deliver skilled, hands-on care: Provide one-on-one therapeutic interventions that improve mobility, balance, and functional ability-directly shaping patient outcomes and quality of life.Educate and empower patients and families: Teach home exercise programs, safety techniques, and ongoing care strategies that encourage progress between visits and help prevent physical therapy setbacks.Monitor progress and communicate changes: Track physical therapy results, adjust treatment plans, and collaborate with physicians and the care team to ensure safe, effective, and timely care.

    Physical Therapist Qualifications
