This Patient-Centered Medical Home offers outpatient primary care services along with integrated behavioral medicine, dental care, podiatry and more. They operate four modern facilities and serve all patients, regardless of economic or insurance status.Become a part of the outpatient primary care team embedded within a critical access hospital in this scenic mountainside community. This organization focuses on relationship-based care, so you can look forward to long patient visits at least 30 minutes, enabling you to provide deep, comprehensive care. Convenient lab and imaging services are available on site.
