    This role serves as a key liaison between patients, providers, caregivers, and operational teams, ensuring effective communication, care plan execution, timely intervention, and escalation of clinical concerns.     Key Responsibilities

    Patient Care Coordination & Monitoring

    Remote Patient Monitoring (RPM) Field Services: Conduct in-home patient visits to deliver, install, configure, educate patients on the use of Remote Patient Monitoring (RPM) devices, verify proper device functionality and connectivity, and retrieve equipment upon completion of the monitoring program. Educate patients and caregivers on the proper use and maintenance of RPM devices.     Key Responsibilities
