RESPONSIBILITIES: * Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing impairments. * Conduct bedside swallowing evaluations and make dietary texture recommendations as appropriate. * Provide direct therapy services and modify treatment based on resident progress. * Document evaluations, daily progress notes, and discharge summaries in accordance with facility policy and payer requirements. * Collaborate closely with nursing, dietary, and therapy staff to ensure coordinated care and optimal resident outcomes. * Educate residents, families, and staff regarding communication techniques, compensatory strategies, and swallowing precautions. * Participate in care plan meetings and contribute to interdisciplinary team discussions. * Maintain compliance with all state, federal, and facility regulations regarding patient care and documentation. * Supervise speech therapy assistants and students as assigned. * Promote a positive, supportive, and therapeutic environment in alignment with the facility's mission and values. * Other duties as assigned.
